At Triad Healthcare Inc., all licensed providers are allowed to join any available network. Therefore, our participating provider network is not restricted or closed to new providers or providers joining existing groups. In fact, we invite highly motivated, quality driven, allied specialty providers to partner with us and demonstrate optimal patient outcomes.

Paperwork can be mailed or faxed to you by contacting us via email at: provider_contact@triadhealthcareinc.com or if you would prefer to call and request paperwork, please call the Provider Service Department at 800-409-9081.

Keep in mind that Triad requires all providers practicing under the same TIN to participate, so, request as many applications as necessary for your office or group.

Online Credentialing and Recredentialing

Triad Healthcare Inc. is pleased to participate in the Universal Credentialing DataSource, a project of the Council for Affordable Quality Healthcare (CAQH). This web-based solution allows you to store and manage your credentialing information online and share it with insurance carriers electronically.

By using this single source, you'll no longer need to complete different credentialing applications for different insurance carriers. Just enter your information once and authorize insurance carriers (including Triad) to access it.

How it works:

• Logon to http://upd.caqh.org
• Enter your CAQH Provider ID if you don't know it, call CAQH at 1-888-599-1771.
• Enter or update your information
