IS YOUR RECREDENTIALING DUE IN 2007?

Please note that Triad Healthcare Inc. is a member of the Council of Affordable Healthcare (CAQH) and a participant of the CAQH Universal Credentialing Database. We are requesting all participating providers to use CAQH for their recredentialing. We are sure that this will improve organizational efficiency and decrease the amount of applications a provider is required to complete overall.

You are a valued provider in the Triad Healthcare IPA,if your recredentialing is due in 2006, we would not want you to be excluded from any contracting initiatives which we are actively pursuing in your area. Your immediate attention to this matter is greatly appreciated.

Log on to the system at www.caqh.org/cred today to either complete, update your credentialing information. If you do not have Internet access, you can call the CAQH Help Desk at 1-888-599-1771 and ask for a paper application to be sent by mail. You can fax the completed paper application to CAQH, at 1-866-293-0414 (toll free).
